Use this checklist when a customer reports a lost title or when you’re cleaning up paperwork before a sale:

  • Alaska duplicate titles are obtained using Form 809, Application for Duplicate Title, submitted to the Division of Motor Vehicles.
  • The owner or authorized agent certifies that the original title has been lost, destroyed, or stolen and provides the VIN, plate, and vehicle details.
  • If a lien is listed on the record, a notarized lien release from the lienholder may be required before a clear duplicate can be issued.
  • Applications can be delivered in person at a DMV office or mailed with the appropriate fee to the central DMV address.
  • Dealers should verify all ownership information and ensure any lien releases are on lender letterhead to meet DMV requirements.
