Idaho duplicate titles are requested through the Idaho Transportation Department (ITD). For a straightforward replacement, owners use the Duplicate Idaho Title Application (ITD 3367) or, in some scenarios, an Affidavit of Lost Title (ITD 3371).

  1. Confirm the vehicle is titled in Idaho and identify the titled owner and any lienholder.
  2. Have the owner or lienholder complete ITD 3367; signatures must typically be notarized or witnessed as instructed on the form.
  3. If ownership or lien changes are involved, ITD may require ITD 3371 instead of a simple duplicate form.
  4. Mail the completed application and fees to ITD or file through the local county motor vehicle office.
