Form 78-006 – Application for Replacement Certificate of Title, with an optional fast-track
version on Form 78-026 if the owner wants quicker turnaround.
Confirm that the vehicle was last titled in Mississippi and identify any active lienholders.
Have the titled owner complete Form 78-006 and mail it to the address on the form with the standard fee.
