Missouri handles lost or damaged titles through the Department of Revenue (DOR) using
Form 108 – Application for Missouri Title and License. You don’t use a special duplicate-only form;
you mark the duplicate box on Form 108 and explain why you need the new certificate.
Confirm the vehicle was last titled in Missouri and pull the current record so you know the owner of record and lien status.
Complete Form 108 in full and check the Duplicate box at the top of the application.
In the duplicate section, state the reason for requesting the replacement (lost, stolen, destroyed, etc.).
Have all owners of record sign the form; notarization and lienholder sign-off may be required in some scenarios.
Submit the form, supporting documents, and fee to a Missouri license office or by mail as instructed.
