Nevada issues duplicate titles through the DMV using Form VP‑012 – Application for Duplicate Nevada Certificate of Title. This form is only for vehicles last titled in Nevada.

  1. Confirm that Nevada is the state of record for the title; if not, the owner must work with the prior state first.
  2. Have the owner of record (or a licensed Nevada dealer with proper authorization) complete and sign VP‑012.
