Utah Duplicate Title & Temporary Tag Guide

Utah‑specific guidance for dealers replacing lost titles and issuing 45‑day temporary permits through the DMV.

Use this checklist when a customer reports a lost title or when you’re cleaning up paperwork before a sale:

  • Utah duplicate titles are requested using form TC‑123, Application for Duplicate Utah Title.
  • The owner certifies on TC‑123 that the original title has been lost, stolen, mutilated, or made illegible and that it has not been endorsed and delivered.
  • A small title fee is required, and the completed form can be mailed to the Motor Vehicle Division or processed through the Motor Vehicle Portal (MVP) when online options are available.
  • If there is an active lien, lienholder information must be correctly reflected; in some cases, a lien release is needed before a clear duplicate can be issued.
  • Dealers should help buyers gather accurate VIN and ownership information and confirm mailing addresses so the duplicate title arrives without issues.
