Duplicate / replacement title workflow

Use this checklist when a customer reports a lost title or when you’re cleaning up paperwork before a sale:

Utah duplicate titles are requested using form TC‑123, Application for Duplicate Utah Title.
The owner certifies on TC‑123 that the original title has been lost, stolen, mutilated, or made illegible and that it has not been endorsed and delivered.
A small title fee is required, and the completed form can be mailed to the Motor Vehicle Division or processed through the Motor Vehicle Portal (MVP) when online options are available.
If there is an active lien, lienholder information must be correctly reflected; in some cases, a lien release is needed before a clear duplicate can be issued.

Use this when the buyer needs to drive on a temporary plate or permit while you finish title and registration work:

When a Utah dealer sells a vehicle to a Utah resident, they typically issue a 45‑day temporary permit.
This 45‑day permit allows the customer to operate the vehicle while the dealer submits paperwork and the DMV issues permanent plates and registration.
Dealers must verify insurance, ID, and ownership documents before issuing the permit and must keep detailed records tied to the VIN and buyer.
The permit expiration date is calculated from the date of sale; customers should be reminded that driving after the 45‑day period without permanent registration is not allowed.
For out‑of‑state buyers, dealers should coordinate with the buyer’s home state, as Utah permits may not always be recognized once the vehicle leaves the state.
