Duplicate / replacement title workflow
Use this checklist when a customer reports a lost title or when you’re cleaning up paperwork before a sale:
Utah duplicate titles are requested using form TC‑123, Application for Duplicate Utah Title.
The owner certifies on TC‑123 that the original title has been lost, stolen, mutilated, or made illegible and that it has not been endorsed and delivered.
A small title fee is required, and the completed form can be mailed to the Motor Vehicle Division or processed through the Motor Vehicle Portal (MVP) when online options are available.
If there is an active lien, lienholder information must be correctly reflected; in some cases, a lien release is needed before a clear duplicate can be issued.
Dealers should help buyers gather accurate VIN and ownership information and confirm mailing addresses so the duplicate title arrives without issues.
