Use this workflow when a customer reports a lost title or when you’re cleaning up paperwork before sale or trade:
Vermont duplicate titles are requested using the Replacement/Duplicate Title application (often referenced as form VT-004 or similar) through the Vermont DMV Title Unit.
The titled owner or lienholder completes the duplicate title application with the correct VIN, owner information, odometer section, and lienholder details.
The form explains that, if a lien exists, the duplicate title will be mailed to the lienholder; if there is no lienholder, it is mailed to the owner/lessor of record.
