Duplicate / replacement title workflow
Use this workflow when a customer reports a lost title or when you’re cleaning up paperwork before sale or trade:
West Virginia duplicate titles are requested on form DMV-4-TR, Affidavit of Duplicate Title for a Motor Vehicle or Watercraft.
The titled owner completes sections for owner information and vehicle information, then selects a reason for the duplicate such as lost, destroyed, or never received.
The DMV-4-TR form must be signed by the owner or legal representative; if someone other than the owner signs, a power of attorney is required.
Completed applications and fees are mailed to the DMV Receiving and Processing unit at the address printed on the form or submitted at a regional office.
Dealers should verify lienholder information, confirm whether the title is being held electronically, and note that penalties can apply for false statements on the affidavit.
